Assisted Living

Assisted Living at Southern Kentucky Senior Living offers independent living with 24-hour staff. We are able to provide laundry, housekeeping, and meal services in addition to medication reminders. This is the facility for those who are just not quite comfortable staying at home by themselves anymore but still are very capable of doing many things for themselves.

The rooms in our facility are much larger and set up in an “apartment” style with indoor and outdoor exits for each room allowing for a more home/independent feel.

Requirements for admission to SOKY Senior Living Assisted Living:

• At least 65+ years of age or older
• Ambulatory or mobile non-ambulatory (Able to transfer self)
• Ability to participate in social activities
• TB skin test or statement from Physician free of communicable disease
• Completed Physician/Healthcare Provider Plan of Care Form
• Flu shot last administered (date)
• Pneumonia shot last administered (date)
• Identification card
• A copy of any Advance Directives or POA

This information listed above is needed 48 hours prior to admission unless the patient is coming directly from the hospital. You can fax clinical information for review to 270-776-9965. All records are reviewed prior to admission to determine appropriateness for assisted living placement.
